Important Information On Student Health Insurance

The student health insurance registration period is Saturday, January 18 through Thursday, February 27

For the 2013-2014 school year, the Student Health Insurance plan will be administered by United Healthcare. Students will now have 24-hour electronic access, 7 days a week to their student insurance coverage information: please access www.uhcsr.com/Myaccount in order to create your account.

During this registration period students may:

-- Exempt from the Student Health Insurance Plan (click on "Waiver Form"

-- Enroll into the plan (click on "Enrollment Form")

-- Add dependents (click on "2013-2 Enrollment Form - Dependent")

-- Reinstate into the plan. You must come in person to the Health Insurance campus office (Mon.-Fri. 9:00 a.m. - noon and 1:00 p.m. - 4:00 p.m., 506 S. Wright Room 100A)
